MZW Agreement
I, {name}, the Client/Student (together with my parent or guardian, if I am under the age of eighteen or under a legal disability) confirm that I am fully aware of my current health and physical condition and am knowingly and voluntarily entering into this agreement. I understand and accept that participating in Strength and Conditioning, Yoga, Martial Arts Training, or Zen Mind/Movement could involve risks that may result in injury, death, property damage, or harm to myself or others. I hereby assume all such risks.

With this understanding, I, along with my heirs, assigns, and legal representatives, waive any and all claims against Terryann DeAngeles, Modern Zen Warrior, its landlord, contractors, instructors, and employees (collectively, the “Released Parties”) for any injuries, damages, losses, or claims, known or unknown, that arise from any activity hosted, sponsored, or facilitated by the Released Parties. I release and forever discharge the Released Parties from any and all claims.

Additionally, I agree to indemnify and hold the Released Parties harmless from any losses, liabilities, costs, or expenses (including but not limited to medical expenses, attorney's fees, and litigation costs) arising from claims brought by me or anyone acting on my behalf as a result of my participation in any activity. I have read and understand this agreement fully and execute it voluntarily.

Session Cancellation Policy

Clients must provide at least 24 hours' advance notice for session cancellations. Notice can be given through a phone call, SMS, or WhatsApp message. If 24 hours’ notice is not given, the session will be forfeited. This policy does not apply in cases of car accidents, hospitalizations, physical injury, or urgent family matters.

Media Consent Agreement
